Supported Living
The supported living service will primarily offer support to adults with a learning and or physical disability to live in and take an active part in their community as a tenant or owner-occupier. This will not preclude such a service being offered to people over 65 who require or would benefit from such a service.
Support will also be provided to adults with learning or physical disability who live with their family as part of a plan to develop independent living skills and an eventual move to supported independent living.
Individuals will be supported to claim all appropriate welfare benefits and thereafter to manage their money effectively.
Citizenship and a community presence will be encouraged and supported. This will ensure people lead rewarding lives as well as contributing to the community they live in.
The Service will adhere to the Reach Supported Living Standards and the enhanced standards by “Changing our Lives” a Sandwell based self-advocacy group for people with a learning disability.
Support, in the form of advice and guidance, will be provided to ensure people maintain their health and keep themselves safe.
Maintaining and making new friendships and relationships will be integral to the service delivery; this includes relationships with family members.
We will regularly consult with all stakeholders regarding quality of services provided. We will use the feedback from the consultation to improve our services.
The service will be flexible in its approach; both in the way the service is delivered and the hours it is delivered.
